A new approach to improving public health has won support from Chester FC .

West Cheshire Clinical Commissioning Group (CCG) has developed 'The West Cheshire Way' to make heath services better and easier for people to use.

The initiative involves GPs, hospitals, social care and mental health and well-being services, and has been launched this week to coincide with Self Care Week 2015.

Blues boss Steve Burr and his squad met up with Dr Huw Charles-Jones, chairman of CCG, commissioning manager Trisha Parker and patient champion Andy Lavender to find out more about it.

The aims of the 'The West Cheshire Way' are to support people to stay well and take responsibility for their own wellbeing, simplify access to different services, reduce hospital admissions by providing more community services to care for people at home, get people back home safely and quickly after a hospital stay and reduce waiting times for GPs, hospital appointments, test results and operations.

CCG hope the support from the football club will help engage with male supporters, who traditionally don't think about health until it is too late.

A series of community events have been organised as part of the launch offering health checks and information about diabetes care, breathing difficulties, stroke prevention, stop smoking services, better mental health and well-being, weight loss, spotting cancers and information about how to stay well and active.

Self Care Week 2015 is between 16 and 22 November and CCG aims to raise awareness about how people can self-care for life.
