A NEW telephone-based NHS service has been promoted at clinics held by Farndon Health Centre.

A health coach, who is normally at the end of a phone line, has attended sessions designed to protect vulnerable people against flu.

The YourHealthCoach initiative is open to patients aged 18 or over in western Cheshire, who are registered at a participating GP practice but is likely to be especially useful to those with ongoing care needs.

Health coaches, who are based in Manchester, are all experienced registered nurses with special training in supporting self-management and can take phone calls from any individual with a health concern.

Nurses will also ring patients to understand and manage their conditions more effectively.

Patients can speak to a health coach, Monday to Friday, between 8am and 8pm, using a free phone number.
